After the emerging era of neurosurgery in the early part of the 20th century by Harvey Williams Cusing (1869-1939), there have been the several notable touchstone points based on the revolutionary advancements in the knowledge of neuroanatomy and surgical instruments. The introduction of surgical microscope in the clinical fields forced the neurosurgeon to dive into the world of cerebral microanatomy and train their hands for completely different techniques. And, the introduction of CT and MR in early 1980’s brought the development of new surgical corridors for the skull base area, which had been regarded as no man’s land. The late 1980’s and early 1990’s were the heydays of microscopic skull base surgery and neurosurgeons believed that they conquered the brain at last. However, the reported outcome of skull base surgery in late 1990’s were too disappointed to recommend those surgical techniques to patients. Therefore, the clinical interest moving to the minimally invasive management for the functional preservation and quality of life sought the new treatment modalities such as radiosurgery and optic instrument like endoscope. At the beginning of 2000’s, the endoscope was regarded as the adjuvant tool for microscope. However, the introduction of endoscopic skull base surgery technique and technical advances in the endoscope and its surgical instruments have brought the new era of neurosurgery.
